The Participant Agreements Packet and YHS Athletic Packet must be completed by anyone participating in the YHS Marching Band Program.  This includes all Marching Band and Color Guard members.  Please print out and complete both of the packets below. They are due no later than 6/10/23.

ATHLETIC PACKET REQUIRES A PHYSICAL AND MUST BE NOTORIZED. We will have a notary available Band Signing Day 6/10/23 but if you can get it notarized before, please do.

Doctor’s Physical is required. If you have one that has not expired yet, please bring a copy. Note you are expected to make sure we have an updated physical on file at all time.

Students will not be admitted to practice or band camp until this packet is completely filled out, signed in all appropriate places, accepted, and on file.  Acceptance of some forms requires payment to be completed.  Some forms must be signed by both parent/guardian and student.
